AFSEC Brings Mishap Investigation Training to Colorado Springs

To bolster the U.S. Space Force's operational readiness and safety posture, the Air Force Safety Center is bringing its Space Mishap Investigation Course directly to the heart of America's space community in Colorado Springs. This iteration of the course, running from August 3-14, 2026, is specifically tailored to empower USSF Guardians and Safety Officers with the critical skills needed to investigate and prevent incidents in the increasingly complex space domain.

Leveraging decades of safety expertise, the Air Force Safety Center has designed the intensive two-week program to provide in-depth instruction on the nuanced art of space mishap investigation. The course focuses on determining the root causes of mishaps by analyzing both human and material factors. By hosting the training in Colorado Springs, AFSEC is ensuring maximum accessibility for the personnel at the forefront of the nation's space mission.

"Bringing this course directly to the heart of space operations is a deliberate effort to empower our Guardians on the front lines of space safety," stated Chris Matchett, Space Safety division chief. "Our goal is to equip them with the analytical tools and investigative mindset necessary to protect our vital national assets and, most importantly, our people."

The curriculum is comprehensive, teaching participants proven investigative techniques, the technical specifics of space and explosive mishaps, and how to clearly and concisely document findings in accordance with DAFMAN 91-222, Space Safety Investigations and Reports. Through a blend of lectures, case studies, and hands-on practical exercises, students will work in teams to dissect realistic mishap scenarios and prepare a formal out-brief of their findings to leadership.

Key subject areas include safety investigation board procedures, systems investigation techniques, human factors, and the unique challenges of space collision investigation.

Eligibility and Registration:

The course is open to military (E-4 to E-7, O-3 to O-5) and civilian (GS-11 to GS-14) Guardians and Space Safety Officers, especially those already serving in a space-related billet. It is highly recommended for professionals in the Space Professional Development Program.
