Junior Reserve Officer's Training Corps & Civil Air Patrol Visit Request Form

This form is only for Junior ROTC and Civil Air Patrol groups.

Thank you for your interest in visiting Space Launch Delta 45 at Patrick Space Force Base and Cape Canaveral Space Force Station, Florida. The World's Premier Gateway to Space. 

Installation locations are geographically separated by approximately 15 miles; therefore, visiting both installations is rare due to time constraints. Tours will be no more than 4 hours in duration. 

IMPORTANT:  Junior ROTC and CAP instructors are responsible for the majority of tour planning at Patrick Space Force Base, Florida. Instructions: 

• You cannot save information on the form and come back to it later. Allow yourself 15-20 minutes to complete.
• Submitting this tour request constitutes that you have read, understand, and agree to the following:

1. Submitting this request does not guarantee my group a tour.
2. I will complete and submit this form no later than 60-days prior to my visit-request date. Visit requests submitted outside of the 60-day requirement will NOT be considered. Tours are not offered from the months Nov. 15 to Jan. 15. 
3. Due to the heat present in Florida during the summer, July and August tour requests are discouraged as to not cause discomfort for visitors.
4. Tours are not scheduled on launch days due to safety policy.
5. Tours are not conducted on Saturdays, Sundays, Federal Holidays, and Family Days.
6. Tours will only be provided between the hours of 8 AM to 4 PM, during duty hours.
7. Tour times are limited to no more than four hours due to mission requirements.  
8. Cadets within the state of Florida have priority.
9. Tours are subject to last-minute cancellation due to mission requirements with little-or- no notice. A back-up plan is highly recommended in the event visit is postponed or cancelled due to mission requirements. 
10. I will provide my own mode of transportation. I will not exceed more than one vehicle. Two vehicles are NOT authorized.
11. I must have more than 25 guests to submit a request and no more than 45 guests in order to be considered for a tour. Must all arrive on one bus.
12. I am responsible for each person in my group and for obtaining each guest's personal information 10 days in advance, required for Installation Access. 
13. In order to be considered for a tour, I must answer each question on this application. I understand that my visit request will not be processed until each question is answered. Unanswered questions will result in a delay and are subject to disapproval.
14. I will spell out each acronym.
15. I confirm each guest is in good health and physical condition.
16. I assume full-financial responsibility for lodging, meals, and other expenses during the tour.
17. Guests must wear long pants and are required to wear closed-toed shoes; high heels are not allowed.
18. Both installations at Patrick-Cape have a Dress Code Policy. JROTC/ROTC/CAP groups/school must wear school-related uniform if participating in a U.S. Space Force/U.S. Air Force sponsored tour. 
19. Historical sites are original and preserved historical property, which means several locations are not equipped for persons with disabilities.  
20. I have read the Photography Policy and will share this information with my group on where we can and cannot take photos while on the installation, and within the vicinity.
21. A parent or Legal Guardian of a minor is encouraged to sign the Authorization for Photographs Involving a Minor Media Release Form (DD Form 2830 General Talent Release), for public release to support the promotion and advertising of US Space Force and US Air Force services programs conducted on Patrick Space Force Base and Cape Canaveral Space Force Station, Florida.
22. I will surrender my camera to a DOD official or Space Launch Delta 45 representative, in the event it is suspected an unauthorized photo was taken.
23. My group will be provided with a safety brief and will comply with all guidelines set forth or my entire group will be escorted off the installation.
24. Guests must be escorted at all times while on the installation by an authorized/badged member. Visitors under the escort of a DOD professional must abide by the instructions and conduct set forth for visits. Visitors must remain in the escorted area on the installation at all times.
25. Smoking, vaping, illegal drugs, firearms, the possession and consumption of alcoholic beverages, are all prohibited while on the installation.
26. Prohibited items include, but are not limited to animals, except service animals for persons with disabilities; Explosives, weapons, and alcohol; Bladed, edged, or sharp weapons; knives with blades 2.5 inches in length or longer; Bicycles, motorcycles, scooters, tricycles, or any other self-propelled or motor-powered personal transport (e.g., Segway) however please note that this criteria does not include medical mobility equipment. Also prohibited are Unmanned Aerial Vehicles (Drones) and oleoresin capsicum (OC) (“pepper spray”).
27. I certify I am 18-years or older.
