Family Health Team wins best in Air Force award

A 45th Medical Group Family Health Team has won a best in the Air Force award.

Each quarter the Air Force Medical Service recognizes its top medical facilities for execution of the military "Patient-Centered Medical Home" model.

The patient-centered medical home is a nationwide initiative in providing comprehensive primary care to adult and pediatric patients by creating a partnership between the individual patient, their personal provider and family members when appropriate.

The provisions of medical homes allow for better access to care, increased satisfaction with care and improved overall health.

Discovery, one of the Patrick Air Force Base Family Health Teams, was awarded "Best  Performing Family Health Team" in the entire Air Force for the second quarter of Fiscal  Year 2011.

They scored a composite score of 99 percent based on the grading parameters of patient continuity, low referral rates to emergency and urgent care clinics, preventative health screenings like cholesterol, diabetes, cervical/breast and colon cancer and, finally, patient satisfaction scores.

The Discovery team is headed by Lt. Col. (Dr.) Paul Doughten and Physician Assistant William Carnes, with a supporting staff to include: Laura Wiler, RN; Heather Skinner, LPN;  Charlene Thomas, LPN; Alversia Williams, LPN; and medical technicians Airman 1st Class Jon Olson and Airman 1st Class Katalina Escobar.

The team was acknowledged at the "Hollywood Squares" Video Teleconference with Lt.  Gen. Charles B. Green, Air Force Surgeon General, and won eight seats to attend the prestigious Disney Institute, a leadership and professional development program offered through the Disney Company.

"We are honored to be recognized by our Air Force Sugeon General for our efforts to continually improve our  delivery of quality care," said Col. Allison Bowden, 45th Medical Group Commander. "It is all about our people and their great teamwork."
